💎 Idea

Romania faces a critical challenge with high rates of preventable and treatable mortality from cardiovascular diseases (CVDs), including coronary heart disease (heart attacks) and cerebrovascular disease (strokes). Many individuals at high risk lack continuous, personalized monitoring outside of clinical settings, leading to delayed identification of worsening conditions and missed opportunities for timely intervention. This gap in proactive, data-driven management contributes significantly to adverse health outcomes and the overall burden of CVDs on individuals and the healthcare system, particularly in urban locations. We aim to bridge this gap via risk prediction based on a multitude of data sources and personalized guidance directly to healthcare providers and their patients.

 Solution

MoniCare is a predictive analytics platform designed for proactive cardiovascular risk management. Targeting private healthcare providers initially, the solution integrates diverse patient data from environmental, behavioral, and clinical sources to provide dynamic risk assessment, personalized insights, and remote monitoring capabilities. The platform empowers patients and enables healthcare providers to intervene proactively, shifting the focus from reactive treatment to preventative care.

Key components include:

• Integration with personal health devices for continuous physiological data collection.
• AI-driven analytics for dynamic CVD risk prediction and early warning based on combined data.
• A healthcare provider dashboard for remote patient oversight, alerts, and data summaries.
• Soon: A dedicated patient application for personalized health monitoring, actionable recommendations, and behavioral change guidance.

🛰️ EU space technologies

MoniCare integrates European space data from the following satellites:

• Copernicus environmental data: Provides vital context by integrating data like air quality (Atmosphere Monitoring Service - CAMS) and temperature/heat risk (Climate Change Service - C3S). This informs personalized risk assessments and enables alerts and WHO-driven recommendations for preventing cardiovascular health deterioration.
• Galileo high-accuracy geolocation (soon): In addition to the prevention layer, we aim to enhance the precision of activity tracking data and enable accurate localization for potential future integration with emergency response systems, supporting faster and more targeted treatment of time-critical CVD incidents.

  EU Space for Healthcare

Leveraging EU Space data to power proactive, data-driven cardiovascular risk management, MoniCare empowers healthcare providers to deliver enhanced care quality and improve patient outcomes. By enabling a data-informed shift towards prevention and early intervention, the platform directly contributes to reducing the concerningly high preventable CVD morbidity and mortality across Romania, supporting the objectives of the Romanian National Health Strategy 2023-2030.
